This MyPhysician360 Pharmacy Compliance Checklist is designed to ensure safe, compliant, and consistent care across all MyPhysician360 pharmacy partners. Pharmacies are responsible for operating in accordance with applicable laws, professional standards, and telehealth requirements. MyPhysician360 provides the platform and support, while pharmacies maintain independent responsibility for dispensing and compliance.

Pharmacies MyPhysician360 network agree to uphold these standards to ensure safe, effective, and compliant patient care.
Section 1: Licensing & Credentials
- Active pharmacy licensure in good standing in all states where the pharmacy dispenses medications, in accordance with applicable state laws (you can use FDA BeSafeRx tools to verify)
- Licensed pharmacists on staff
- Compliance with state and federal regulations
- Adherence to USP standards (<795>, <797>, <800>)
- For compounding pharmacy, confirm PCAB accreditation status
Section 2: Compounding & Sourcing
- Transparent sourcing of all compounded medications
- 503A (in-house) or 503B (outsourced) disclosure
- APIs sourced from FDA-registered suppliers
- COAs available upon request
- Batch and sourcing documentation maintained
Section 3: Prescription Standards
- Valid telehealth consultation required
- Pharmacist review prior to dispensing
- Prescription appropriateness verified
- No unauthorized substitutions or changes
Section 4: Ingredient Compliance
- Only approved ingredients used
- Avoid restricted or unsafe substances
- Align with MyPhysician360 guidelines
- Participate in compliance reviews if flagged
Section 5: Documentation & Safety
- Maintain prescription and compounding records
- Ensure proper labeling and patient instructions
- Provide counseling when required
- Maintain HIPAA compliance
Section 6: Communication
- Respond to inquiries within SLA
- Participate in compliance reviews
- Report regulatory or licensing changes
- Maintain open communication with our team
